Magnesium in Perimenopause: Could This One Mineral Be Behind Your Poor Sleep, Anxiety, Leg Cramps and More?

Have you ever wondered if some of the symptoms you're experiencing are "just perimenopause"... or if your body is trying to tell you something else?

You finally crawl into bed after another busy day. You're exhausted, but sleep doesn't come easily. When you do drift off, you wake at 3 a.m. with your mind racing. Your calf suddenly cramps as you stretch your legs. Your eyelid has been twitching for days and your shoulders feel permanently tense.

Sound familiar?

Many women assume these are simply inevitable parts of getting older or going through perimenopause. While fluctuating hormones certainly play a role, there is another surprisingly common factor that often flies under the radar: magnesium.

As a nutritional therapist, magnesium is one of the nutrients I discuss most often with women in their forties and fifties. Not because it's a miracle supplement, but because modern life, chronic stress and hormonal changes all increase our need for this remarkable mineral.

And when we don't have enough, our bodies often let us know.


Why magnesium becomes even more important during perimenopause

Magnesium is involved in more than 600 enzymatic reactions throughout the body. Every heartbeat, every muscle contraction, every nerve signal and every molecule of energy your body produces depends on it.


But here's what makes magnesium especially relevant during perimenopause.

Hormones are fluctuating.

Sleep often becomes lighter.

Our soils and therefore foods we eat have less minerals.

Stress levels tend to increase as many women juggle careers, ageing parents, teenagers, relationships, fluctuating hormones and changing bodies.

Blood sugar becomes less stable.

Migraines may become more frequent.

Muscles often feel tighter and recovery takes longer.

Every one of these processes increases your body's need for magnesium.


One of the biggest patterns I see is that women don't suddenly become magnesium deficient because they enter perimenopause. Rather, perimenopause often exposes a deficiency that has been quietly developing for years.


Stress and magnesium: a vicious cycle

If you've been following my work for a while, you'll know I often talk about becoming a Stress Detective.

Magnesium fits perfectly into that picture.

When we're under stress, our bodies burn through magnesium faster. At the same time, low magnesium makes our nervous system more reactive to stress.

The result?

Stress uses magnesium → lower magnesium makes us feel more anxious → poor sleep follows → our stress response becomes even stronger.

It's a vicious cycle.

Breaking that cycle often requires more than simply taking a supplement, but restoring magnesium can be one important piece of the puzzle.


Could your symptoms be linked to low magnesium?

Magnesium deficiency doesn't always show up on a routine blood test, but it often shows up in the way you feel.

You may recognize yourself if you experience:

  • headaches or migraines

  • muscle cramps or tight muscles

  • restless legs

  • eye twitching

  • poor sleep or waking during the night

  • anxiety or feeling "wired but tired"

  • constipation

  • chocolate cravings

  • fatigue

  • muscle soreness after exercise

  • heart palpitations or an awareness of your heartbeat

  • elevated blood pressure

  • PMS symptoms

  • neck and shoulder tension


Of course, these symptoms can have many different causes. Magnesium isn't the answer to everything - but it is one of the first nutrients I think about when several of these occur together.


A simple conversation that made me smile

Recently, a friend mentioned that she used to wake regularly with painful leg cramps.

I asked whether she'd started taking magnesium.

She smiled and replied, "Actually, yes. I hardly ever get them anymore."

Now, that doesn't prove magnesium was the only reason her cramps improved. But muscle cramps are one of the classic signs that muscles may not be getting enough magnesium to relax properly.

Sometimes the simplest changes really can make a noticeable difference.


Why are so many women low in magnesium?

Many people assume they eat reasonably well, so how could they possibly be deficient?

Unfortunately, there are several reasons.

Modern diets often contain fewer magnesium-rich whole foods than they once did. 

Our soils are depleted and therefore the foods we eat contain much less magnesium than they used to. 

Chronic stress continually increases our requirements.

Digestive problems such as low stomach acid, IBS or inflammatory bowel conditions may reduce absorption.

Certain medications - including proton pump inhibitors, some diuretics and other commonly prescribed medicines - can also affect magnesium status.

Heavy sweating, regular alcohol consumption and high sugar intakes may further increase losses.

And during perimenopause, our overall demand often rises just as many women are sleeping less and feeling more stressed.

It's not usually one single factor.

It's the combination.


Can you get enough magnesium from food?

For many women, food alone may not be enough to fully meet their magnesium needs.

Modern farming practices and changes in soil quality have contributed to lower mineral levels in some crops compared with previous decades, although the extent varies depending on the food and where it is grown. At the same time, chronic stress, hormonal changes, lower digestive function and increased physiological demands during perimenopause can all increase your body's need for magnesium or reduce how much you absorb.


In other words, it's not just about how much magnesium is in your food - it's also about how much your body needs and how much it can actually absorb and retain.

That's why I often see women who eat a nutrient-rich diet but still benefit from magnesium supplementation, particularly during periods of high stress or hormonal change.

Excellent food sources of magnesium include:

  • dark leafy greens

  • pumpkin seeds

  • sunflower seeds

  • almonds and cashews

  • avocado

  • black beans and lentils

  • cacao and dark chocolate (min 70%)

  • liver and other organ meats


However, correcting a significant deficiency through food alone isn't always easy.

Many women simply aren't eating these foods regularly.

Others have digestive issues that reduce absorption. Magnesium, like many minerals, depends on healthy stomach acid and a well-functioning digestive system to be absorbed efficiently.

That's one reason why I often support clients with digestive function,  dietary changes and carefully chosen supplements.


Not all magnesium supplements are the same

This is probably one of the biggest misconceptions I see.

Many women tell me,

"I tried magnesium but it upset my stomach."


The problem often isn't magnesium itself.

It's the form they were taking.


One client came to see me convinced she had developed serious digestive problems. She was experiencing loose stools almost every morning and assumed something was wrong with her gut.

When we reviewed her supplements, we discovered she was taking magnesium citrate before bed because she'd heard magnesium could help her relax.

She was absolutely right about magnesium helping relaxation.

But magnesium citrate is not well absorbed and also pulls water into the bowel that's why it is commonly recommended for constipation.

In her case, the supplement - not her gut - was causing the diarrhea.

We switched her to magnesium glycinate, and within a short time her digestion settled.

It's a perfect example of why choosing the right form matters.


Which type of magnesium is right for you?

Different forms of magnesium are bound to different compounds, which affects how well they're absorbed and the situations in which they may be most helpful. The "best" form depends on your symptoms and your health goals.

Magnesium glycinate (or bisglycinate)

Best for: Sleep, relaxation, anxiety, muscle tension, PMS and restless legs.

This is my first choice for most women in perimenopause. Magnesium glycinate is one of the best-absorbed forms and is gentle on the digestive system because it's bound to the amino acid glycine, which also has calming properties.


Magnesium glycerophosphate

Best for: Everyday magnesium supplementation, long-term replenishment and sensitive digestion.

Like glycinate, magnesium glycerophosphate is very well absorbed and is usually well tolerated. It's an excellent choice if your primary goal is simply to restore magnesium levels over time without causing digestive upset.


Magnesium malate

Best for: Muscle fatigue, chronic aches, energy production and recovery.

Malate plays a role in energy production, making this form a good option if fatigue or muscle soreness are your main concerns. Because it can feel slightly energising, many people prefer taking it earlier in the day.


Magnesium taurate

Best for: Heart health, blood pressure and cardiovascular support.

This form combines magnesium with the amino acid taurine, which supports healthy heart rhythm and vascular function. It's sometimes recommended for people experiencing heart palpitations or elevated blood pressure, although any persistent cardiac symptoms should always be discussed with your healthcare provider.


Magnesium threonate

Best for: Brain health, memory, focus and cognitive support.

Magnesium threonate appears to cross the blood-brain barrier more effectively than other forms, making it an interesting option for brain fog, memory and cognitive function. Some women also find it supports relaxation and sleep.


Magnesium chloride

Best for: Muscle relaxation and topical use.

Commonly found in magnesium oils, sprays and bath flakes. Some people find topical magnesium helpful for sore or tight muscles, particularly if digestive issues make oral supplements difficult to tolerate.


Magnesium sulfate

Best for: Relaxation through Epsom salt baths.

Magnesium sulfate is the form found in Epsom salts. While soaking in an Epsom salt bath can be wonderfully relaxing and soothing for tired muscles, there is still limited evidence that significant amounts of magnesium are absorbed through the skin. I tend to recommend it for the relaxing ritual and muscle comfort rather than as a primary way to correct magnesium deficiency.


Magnesium citrate

Best for: Constipation.

Magnesium citrate draws water into the bowel, making it very effective for relieving constipation. However, if you're looking for support with sleep, stress or muscle tension - and your digestion is already normal - this probably isn't the form I'd choose.


Magnesium oxide

Best for: Occasional constipation (not replenishing magnesium stores).

Although magnesium oxide contains a large amount of elemental magnesium, it is one of the least well-absorbed forms. It's inexpensive and therefore commonly used in many supplements, but if your goal is to improve your magnesium status, there are much better options available.


What about combination magnesium supplements?

You don't have to choose just one form.

Many high-quality magnesium supplements combine several well-absorbed forms to provide broader support. For example, a product might include glycinate for relaxation, malate for muscle function and energy, and glycerophosphate for general replenishment.

These combination products can work very well - as long as they are based on well-absorbed forms rather than relying mainly on cheaper ingredients like magnesium oxide.

The most important thing isn't finding the "perfect" form. It's choosing a high-quality supplement that uses bioavailable forms of magnesium and matches your individual needs.


Can a blood test tell if you're deficient?

This is a question I hear regularly.

Surprisingly, not always.

Only a very small percentage of the body's magnesium circulates in the blood. Most of it is stored inside cells, bones and tissues.

That means it's entirely possible to have a "normal" serum magnesium result while your tissues still aren't getting enough.

That's why I never look at a blood test in isolation. Your symptoms, diet, medications, stress levels and digestive health all provide valuable clues.

If you do test magnesium in your blood, you need to test the cellular magnesium (RBC).


Magnesium doesn't work alone

One of the themes I've been writing about recently is mineral balance.

Magnesium is incredibly important - but it doesn't work in isolation.

It relies on adequate sodium, potassium and vitamin B6 to function optimally inside the cells. Healthy digestion also matters because minerals need to be absorbed before they can do their job.

This is why I'm rarely interested in asking, "Which supplement should I take?"

The better question is,

"Why does my body need more magnesium in the first place?"

Is stress increasing your requirements?

Is poor digestion reducing absorption?

Are hormonal changes affecting your needs?

When we answer those questions, supplementation becomes far more effective.


The bottom line

Magnesium isn't a miracle cure, but it is one of the most important nutrients for supporting the nervous system, muscles, sleep, energy production and hormone balance - especially during perimenopause.


If you're struggling with poor sleep, muscle cramps, anxiety, headaches, constipation or heart palpitations, magnesium may be one piece of the puzzle.

But it's rarely the whole story.

The real goal isn't simply to take another supplement. It's to understand why your body is asking for more support in the first place.
